Membership Terms & Conditions
Membership
- Membership is valid for 12 months from the date of enrollment.
- Benefits are non-transferable and may only be used by the enrolled member.
- Included services and discounts expire at the end of the membership period and do not roll over into future years.
- Memberships are non-refundable.
Payment & Discounts
- Payment is due at the time of enrollment.
- The 10% member savings does not apply to Invisalign treatment or retail products (including Prevident, whitening refill kits, and similar items).
- Membership discounts cannot be combined with any other discounts, promotions, or dental insurance benefits.
- Membership fees may not be financed through third-party financing programs, including CareCredit.
- Patients may use CareCredit or other third-party financing for treatment; however, member discounts do not apply to treatment financed through third-party financing.
Scheduling
- Members are responsible for scheduling appointments within their membership year.
- Benefits expire after 12 months regardless of whether all included services have been used.
Annual Renewal
- Membership renews annually on the anniversary of enrollment.
- A renewal reminder and payment invoice will be sent by text and/or email approximately 30 days before the renewal date.
- Benefits continue without interruption when renewal payment is received on or before the renewal date.
Lapsed Membership
- If renewal payment is not received by the expiration date, membership benefits immediately terminate.
- Membership may be reinstated within 60 days for a $50 reinstatement fee.
- After 60 days, a new membership enrollment is required.
- The Schiff & Moniz Dental Group reserves the right to modify membership fees or terms upon annual renewal.
