  • Membership excludes Invisalign treatment
  • Discount does not apply to products (Prevident, bleaching refills, etc.)
  • Cannot be combined with any other discounts or promotions
  • Payment is due at the time of service
  • Membership fees and terms are subject to change upon annual renewal
  • Membership benefits are non-transferable and may only be used by the enrolled member
  • Membership is valid for 12 months from the date of enrollment
  • Included services and benefits expire at the end of the membership period and do not roll over into future years
  • Members are responsible for scheduling appointments in advance
  • Membership benefits expire after one year regardless of appointment availability or unscheduled treatment
  • Membership does not renew automatically
  • Patients may renew annually at the current membership rate and terms at the time of renewal
